Forms for Students Enrolled
in Faculty of Sciences Programs
- Course Add Form
To be used by students only when adding courses after the last day to add online. It will require a course coordinators signature before it can be processed by the Faculty Office.
- Course Drop Form
This form is only for use be students who are not able to drop classes online eg: Cross-Institutional students.
- Credit Transfer
Information regarding applying for credit for previous tertiary studies.
- Non Award Study
Information on enrolment procedures for Non Award Study
- Pre-Requisite or Co-Requisite Waiver Form
Students intending to apply for a waiver for a course with a
co-requisite or pre-requisite are required to fill out this form.
Students are required to fill out a single form for each course.
- Student Leave of Absence Form
This policy addresses the need for the University of Adelaide to better manage student load in response to provisions of the Higher Education Support Act 2003 (HESA) that allocate funding only for approved Commonwealth Supported Places.
- Third Time Enrolment Form
This form is for students who have twice failed a course, and wish to enrol a subsequent time.
- Supplementary Exam Forms
All forms and polices relating to exams can be found on the Examinations web site. Note that requests for supplementary exams on Compassionate grounds must be accompanied by appropriate supporting documentation. The Faculty provides guidelines on what is considered acceptable.
